- Signature at dept.: Signature before leaving the school.
- Time of arrival.: For short leave teacher must come back to school after leave-if leave is availed in the morning. Write time when teacher is back to school.
- Signature at arr.: Signature after coming back to the school.
- Head Teacher: Signature of the head teacher.
